Set your Google Form ID

In this file, set ERMITS_GOOGLE_FORM_ID to the id from your form URL (the segment after /d/e/ and before /viewform).

Optional: set ERMITS_GOOGLE_FORM_EMAIL_ENTRY (e.g. entry.1507904553 from Get pre-filled link) so ?email= prefill works.
